COMPLETE THE FOLLOWING STEPS (in either order)

• Come Visit Us!  Call Dr. Rob Newcomer at 580-559-5397 or to schedule a day & time to visit.

• Complete a Form of Declaration of Major or Minor or
  Change of Major or Minor
