Employee Benefits - Life Insurance
Basic Life and AD&D Highlights -- University Paid Benefit
- 2 times your Annual Earnings, rounded to the nearest dollar. Maximum of $250,000. (Annual salary updated each January 1)
- AD&D is equal to the amount of your Basic Life. Also, AD&D includes a Drug and Alcohol exclusion.
- Includes Child Care, Higher Education and Career Adjustment Benefits
- Child Care Benefit: The total child care expense incurred by your Spouse within 36 months after the date of your death for all children under age 13, but not to exceed $5,000 per year, or the cumulative total of $10,000 or 25% of the AD&D Insurance Benefit, whichever is less.
- Higher Education Benefit: The tuition expenses incurred per child within 4 years after the date of your death at an accredited institution of higher education, exclusive of board and room, books, fees, supplies and other expenses, but not to exceed $5,000 per year, or the cumulative total of $20,000 or 25% of the AD&D Insurance Benefit, whichever is less.
- Career Adjustment Benefit: The tuition expenses for training incurred by your Spouse within 36 months after the date of your death, exclusive of board and room, books, fees, supplies and other expenses, but not to exceed $5,000 per year, or the cumulative total of $10,000 or 25% of the AD&D Insurance Benefit, whichever is less.
- A Repatriation Benefit is included. This benefit provides an additional benefit to help pay for the expenses incurred by the transport of an insured employee's body to a mortuary near your primary place of residence, when the employee dies more than 200 miles from home; not to exceed $5,000 or 10% of life insurance benefit, whichever is less.
- A Seat Belt and Air Bag Benefit are included: Seat belt will now be the lesser of (1) $10,000; or (2) the amount of AD&D Insurance Benefit payable for loss of life. Air Bag will now be the lesser of (1) $5,000; or (2) the amount of AD&D Insurance Benefit payable for Loss of your life.
- An Accelerated Benefit up to 75% of Life and Supplemental Insurance is included.
- MEDEX Travel Assist is included, offering plan participants access to appropriate medical care and other emergency services when traveling at least 100 miles from home or internationally. MEDEX Travel Assist offers a full range of professional 24-hour medical, legal and travel assistance services.
- Portability of Insurance is included for active employees.
- Annual Earnings exclude commissions.
- Reduction of Insurance: Life and AD&D:
- Age 65 through 69 will be 65%
- Age 70 through 74 will be 35%
- Age 75 or over will be 25%
- If you are an active member and you reach an age shown above, the amount of insurance will be the amount determined from the Schedule of Insurance, multiplied by the appropriate percentage above.
Additional Life Highlights -- Out of Pocket Expense
- The policyholder will be notified of renewal rates 60 days prior to the renewal effective date.
- Active Additional Life rates are guaranteed for three years.
- Waiver of Premium is included. Eligibility to age 60; ends at age 65.
- MEDEX Travel Assist is included. (see above)
- An Accelerated Benefit of up to 75% of Life and Supplemental Insurance is included.
- Portability of Insurance is included for active employees.
- Proof of good health is required for employees who enroll more than 31 days after eligibility.
- Proof of good health is required for elective increases.
- Until coverage has been in force for 2 years, death which results from suicide or other intentional self-inflicted injury is not covered.
Dependent Life Highlights -- Out of Pocket Expense
Currently there has been only one choice for qualifying dependents ($2.42/month premium rate) of $10,000 spouse, dependent child of 6months or more $500 or dependent life birth to 6 months $500. New dependent life offers 3 options. See rate sheet for details. If you currently have dependent life you must re-enroll to continue.
